How's it going?
A quick check-in for when you're starting ADHD medication, or it's being reviewed. A minute a day to notice how you're doing, so you have something real to tell whoever prescribes it.
It's yours. Your answers save on this phone only, nobody else can see them, and you choose whether to show anyone.
I'm a coach, not a clinician.
This is for noticing, not deciding. It isn't medical advice and it doesn't replace your prescriber. Please don't change, skip or stop a dose because of what you see here. Talk to your prescriber or a parent or carer first.
If you're worried about how you feel on your medication, tell an adult you trust, your prescriber or your GP. If it's urgent, ring 111, or 999 in an emergency. You can also text SHOUT to 85258 or call Childline on 0800 1111, free, any time.
Why bother?
- Finding the right medication takes time. It can take a few tries at the medication and the dose. If one isn't helping, that doesn't mean none will.
- Appointments are short. “How was it?” is hard to answer on the spot. A week of notes is easier, and clearer.
- “Did I feel like me?” is the question that gets missed. Some people feel flat, quiet or far away on medication. If that's you, it's worth saying.

Parents and carers
It works best if your teen keeps it themselves and decides what to share. You can offer to look through the week together before the appointment, and add anything you've noticed in the notes box. Please don't read it without asking. For younger children, the printable diary is designed to be filled in together.
